More Than 70 Percent of Consumers Can Find Marketplace Plans for Less Than $75 per Month

October 31, 2016 By HHS Press Office

This post was originally published on HHS.gov, October 24, 2016

With window shopping beginning today, Health Insurance Marketplace consumers can now visit HealthCare.gov to check out their options for 2017 coverage in advance of the start of Open Enrollment on November 1. A new report released today shows that 72 percent of Marketplace consumers in states using HealthCare.gov will be able to find plans with a premium of less than $75 per month and 77 percent will be able to find plans with premiums below $100, taking into account financial assistance. The report also shows that consumers will have options, with an average of 30 health insurance plans to choose from.

“Thanks to financial assistance, most Marketplace consumers this year will find plan options with premiums between $50 and $100 per month,” said HHS Secretary Sylvia M. Burwell. “Millions of uninsured Americans qualify for financial assistance, and so could as many as 2.5 million Americans currently paying full price for off-Marketplace coverage. I encourage anyone who might need 2017 coverage to visit HealthCare.gov and check out this year’s options for yourself.”
Thanks in large part to the Marketplace, in early 2016, the share of Americans without health insurance fell to 8.6 percent, the lowest level in our nation’s history. This year’s Open Enrollment offers the chance to build on that progress and further improve access to care and financial security.

Financial Assistance and Shopping Help Keep Coverage Affordable

Eighty-five percent of current Marketplace consumers receive tax credits that bring down the cost of coverage, and, nationwide, about the same percentage of Marketplace-eligible uninsured Americans also have incomes that could qualify them for tax credits. In addition, an estimated 2.5 million people currently paying full price for health insurance in the off-Marketplace individual market could be eligible for tax credits if they purchase 2017 coverage through the Marketplace.
Tax credits increase dollar-for-dollar with the cost of a consumer’s benchmark plan, so they protect the large majority of consumers from rate increases. For example, a 27-year-old in Dallas, Texas with an income of $25,000 paid $143 per month to purchase the benchmark (second-lowest cost silver) plan in 2016. For 2017, that same 27-year-old would pay almost the exact same amount for benchmark coverage, despite a premium increase of $16 per month, because tax credits also increase to compensate.

In addition to financial assistance, shopping helps keep coverage affordable for consumers. If every returning consumer nationwide selected the lowest-cost plan within the same metal level they picked last year, average premiums paid (taking into account financial assistance) would fall by $28 per month – 20 percent – compared to 2016. In fact, many consumers do not choose the lowest-cost plan available, which is why it’s important that shopping also lets consumers compare plans based on factors like provider network, prescription drug coverage, and total out-of-pocket costs. But this calculation confirms that affordable options for 2017 coverage are available to consumers who choose to shop around to find a better deal.

This year, HealthCare.gov consumers will have the option to choose from an average of 30 plans. Like last year, there will be an average of 10 plans per issuer. Four out of five (79 percent of) consumers will also be able to choose between multiple issuers, and all consumers will be able to choose among plans with different combinations of premiums, out-of-pocket costs, networks of hospitals and physicians, and prescription drug coverage options. For people with employer-sponsored health insurance, plan choice is typically narrower; for example, in 2015, 30 percent of people with employer coverage had only one plan option.

As the Marketplace Goes Through a Transition Year, Experiences Vary Widely Across States

Through 2016, Marketplace rates remained well below initial projections from the independent Congressional Budget Office, and well below the cost of comparable coverage in the employer market. Nationwide, average Marketplace premiums for 2017 are increasing more than they have in the past two years. Even with these adjustments, premiums remain roughly in line with projections issued by the Congressional Budget Office during the debate over the Affordable Care Act (ACA).

Adjustments this year reflect issuers bringing their rates in line with observed costs, now that two years of data are available. In addition, some of the ACA’s programs designed to support the new market in its early years are ending this year, putting additional one-time pressure on premium growth. Issuers are continuing to adapt to a new market that looks very different than it did before the ACA: one where they compete based on price and quality, rather than by finding the healthiest customers. Efforts to undermine the ACA, such as certain states’ decisions not to expand Medicaid and Congressional actions to block funding for the law, contribute to higher premiums as well. And some states have long faced unique challenges in reining in health care costs.

For the median HealthCare.gov consumer, the benchmark second-lowest silver plan premium is increasing by 16 percent this year, before taking into account the effects of financial assistance; that is, half of HealthCare.gov consumers are seeing increases less than, and half greater than, 16 percent. But experiences across states vary widely. For example, in Arkansas, Indiana, Michigan, Nevada, New Hampshire, New Jersey, North Dakota, and Ohio, as well as in California and Massachusetts (which do not use the HealthCare.gov platform), benchmark premiums are rising by 7 percent or less. In most of these states, Marketplaces are also strongly competitive, and several have seen issuers expanding their service areas, as with Molina in Ohio or Humana in Michigan. These examples illustrate that there are parts of the country where Marketplaces are already maturing, reaching stable price points, and enjoying robust competition.

Conversely, some states are experiencing high benchmark premium growth, resulting in a HealthCare.gov average higher than the median increase. A number of the states in this group, including Arizona, Hawaii, Illinois, Kansas, and Pennsylvania, are places where 2016 rates were especially far below the national average or especially far below the cost of comparable coverage in the employer market.

Fortunately, financial assistance and the ability to shop around for coverage protect most consumers across the country from headline rate increases. “Before the ACA, many consumers were unable to get health coverage at all, and the individual market offered no easy way to shop and compare plans,” said Kevin Counihan, CEO of the Health Insurance Marketplace. “Because of the Marketplace, consumers can shop around to find coverage that fits their needs and get tax credits to help pay for it. Thanks to shopping and financial assistance, consumers will continue to have robust options for quality, affordable coverage for 2017, even in places where premium increases are high.”

The Affordable Care Act Is Strengthening Coverage, and Improvements Could Drive Further Progress

The Marketplace is one of many ways the ACA continues to improve health care affordability, access, and quality.

  • Affordability. The latest available data show employer premiums are continuing to rise at the low rates observed since the ACA was enacted. This year, the average family premium for the more than 150 million Americans with employer coverage is $3,600 lower than it would be if pre-ACA premium growth had continued.
  • Access. More than 20 million American adults have gained coverage as a result of provisions of the Affordable Care Act. In addition, more than 3 million children have gained coverage since 2008, thanks in large part to the ACA and improvements to the Children’s Health Insurance Program.
  • Quality. For those who already had coverage, the ACA provided new protections, including guaranteed limits on out-of-pocket costs, no limits on annual or lifetime coverage, and preventive services without cost sharing. The ACA is also strengthening health care quality, with large drops in patient harms and preventable hospital readmissions for Medicare beneficiaries.
“Our nation has made historic progress under the ACA, and now we want to build on that progress to further improve affordability, access, and quality,” said Secretary Burwell.

The President has taken a number of steps to improve the ACA, and thinks we can do even more, such as: expanding Medicaid in states that have declined to do so; providing more tax credits for middle-income families and young adults to further improve affordability; adding a public plan fallback to give people more options in places where there still are just not enough insurers to compete; and supporting innovation by states. But this Open Enrollment, we are focused on getting as many uninsured people covered as we can.

Open Enrollment, which starts November 1 and ends on January 31, 2017, provides another opportunity for Americans to find affordable coverage for them and their families. Learn more on HealthCare.gov.

Senator James Lankford and Representative Steve Russell

Two Oklahoma Congressmen up for RE ELECTION this YEAR
BOTH CONSERVATIVE REPUBLICAN
BOTH MEN OPPOSE ALL THINGS LGBTQ RELATED

David Moorman
10/31/2016

________________________________________________________________________________

Once again I have heard back for these two men on an issue I sent into their offices.

James Lankford - NO RESPONSE only a form letter stating my letter was received.

Steve Russell - RESPONSE that of course had NOTHING to do with the letter I sent him.

Both of these men have CONSTANTLY OPPOSED all LEGISLATION that had anything to do with the LGBTQ COMMUNITY. THEY both have made it very CLEAR that they have utter disgust for people LIKE ME who are LGBTQ.

The most recent HATEFUL thing STEVE RUSSELL has done was try to SNEAK in an AMENDMENT to the Defense Budget.

A necessary defense bill has become a loaded issue after Republican lawmakers attached a provision that allows federal contractors to discriminate against employees based on their sexual orientation or gender identity.

The National Defense Authorization Act (NDAA) is a very basic federal law which creates the budget for the Department of Defense. It is reauthorized every year, along with any amendments that get attached to it. However, this year the NDAA has taken on a more controversial role, due to the attached Russell Amendment that many see as legalizing discrimination against the LGBT community.

The Russell Amendment, named after it was introduced by Representative Steve Russell (R-Oklahoma), was attached to the NDAA in the middle of the night and was passed by the House without a hearing, NBC reported.

In a letter to the House and Senate Armed Services Committees, 42 Democratic senators urged the committee to remove the section, saying it “would vastly expand religious-affiliated organizations receiving federal funds to engage in discriminatory hiring practices.”

It is not only LGBT employees whose jobs would be at risk due to the Russell Amendment. The letter explains that the amendment “would also allow religiously-affiliated contractors and grantees to inquire about and discriminate against employees or potential employees based on an individual’s religion.”

The language of the amendment “would give legal grounds for an employer to prevent a gay man from having their husband added to their health insurance,” David Stacy, government affairs director for the Human Rights Campaign, explained to NBC. “Trans people could be fired from their job upon announcing their intent to transition."

Senate Democrats are pushing for Republicans to drop the provision and have reached out to Chairman of Senate Armed Services Committee John McCain (R-Arizona) to remove the provision from the Senate version of the NDAA.

Stacy encouraged McCain to “stand with the majority of fair-minded Arizonans – and Americans all across the country – and drop this discriminatory provision,” The Washington Post quoted.

However, the likelihood of McCain crossing party lines for the issue is unlikely to happen anytime soon. He was quoted by NBC as saying, "I think it's pretty obvious that we're going to have to go to lame duck," meaning it would not be resolved until after Election Day.

Democrats seem to be prepared to put up a fight over the religious liberties act. While no Democrat has outright threatened to filibuster the NDAA if the Russell Amendment is included, Senator David Blumenthal (D-Connecticut) told NBC: "We will have to consider all the options.”
However, supporters of the amendment claim that military chaplains need the Russell Amendment in order to use vendors that meet their religious standards. But if President Barack Obama’s 2014 executive order that prohibits federal contractors from discriminating against LGBT workers is not given a religious exemption, chaplains would be forced to disregard their religious beliefs while choosing vendors for prayer rugs, music, and other tools they use.

However, the Russell Amendment is not limited to military chaplains, especially because out of the more than 1.4 million active military members, less than 3,000 are military chaplains.

In this letter you can see that I was asking both these men to work with the rest of Congress to ensure that funding for the Housing Opportunities for Persons with AIDS {HOPWA} does not get cut. The letter was concerning PEOPLE LIVING WITH HIV/AIDS. You can read the letter here and then read the response I received below from Rep. Steve Russell.

Letter to Sen. James Inhofe, Sen. James Lankford and Rep. Steve Russell

In Rep. Steve Russell's response you will NOT FIND any MENTION of HOPWA. You want to know why? It is because Rep. Steve Russell does NOT care for PEOPLE living with HIV/AIDS. Rep. Steve Russell is one of the Conservative Republicans that believe ONLY GAY MEN get HIV/AIDS. Rep. Steve Russell needs to be VOTED out of OFFICE on November 8th along with Sen. James Lankford. OKLAHOMA NEEDS CONGRESSMEN who will REPRESENT ALL OKLAHOMANS and NOT USE THEIR RELIGION TO WRITE BILLS TO DISCRIMINATE AGAINST people like me [HIV POSITIVE and GAY] and the rest of the LGBTQ COMMUNITY in OKLAHOMA.

Here is the letter I received back from Rep. Steve Russell. Oh yeah remember I receive only a FORM LETTER from Sen. James Lankford saying he received my letter. I almost forget to mention that my letter went to Sen. James Inhofe too and I got NO RESPONSE at all from his office as of today 10/31/2016. Again, Sen. Inhofe is a Conservative Republican that has VOTED AGAINST ALL THINGS concerning the LGBTQ COMMUNITY so I can only image what kind of response I will get over this letter, if I get one. I have written several letters to these men and everytime I get a FORM LETTER back from Sen. James Lankford saying he will be in touch soon. I have NEVER heard back from Sen. Lankford. I just figured it is because he KNOWS that I am GAY and he does not want anything to do with the LGBTQ COMMUNITY.

Letter from Rep. Steve Russell

As you can see once again Rep. Steve Russell has NOT addressed my CONCERNS over the HOPWA budget cuts. Rep. Steve Russell and Sen. James Lankford have both once again proven to me that they DO NOT REPRESENT me or the LGBTQ Community of OKLAHOMA and therefore they MUST GO!!

I will be be VOTING ALL BLUE ALL NEW on NOVEMBER 8th and I pray that everyone who agrees with me that the LGBTQ COMMUNITY does NOT deserve to be DISCRIMINATED against will VOTE BLUE ALL NEW in CONGRESS TOO!!!

Thank you and God Bless America,
David Moorman

Trump Again Tries to Pretend He’s a Friend to LGBT Voters, Enters Rally with Rainbow Flag: WATCH

by Andy Towle
October 31, 2016

_________________________________________________________________________________

Donald Trump once again tried to pretend that he’s a friend to LGBT people, entering a rally in Greeley, Colorado on Sunday night clutching a rainbow flag emblazoned with ‘LGBT for Trump’ which he unfurled and showed to the crowd.

Here’s the truth, via Chad Griffin:

Donald Trump has spent his campaign threatening to undo crucial protections for LGBTQ people in the United States, vowing to appoint judges to the Supreme Court to turn back the legalization of same-sex marriage and supporting state-sanctioned discrimination against transgender people.

He has courted extremists who foment hate both here and export it abroad. His running mate, Mike Pence, as a congressman in 2009, opposed language in federal legislation to monitor and combat human rights violations against LGBTQ people abroad, accusing Democrats of attempting to “promote a gay rights agenda around the globe.”

Trump has also sought support from leading exporters of anti-LGBTQ hate, including the Alliance Defending Freedom. Well known for the work it does within US borders, ADF is also spreading hate in countries around the world, from fighting equality in Russia to recruiting and training anti-equality activists in Nepal.

Trump also has an advisory board chock full of anti-LGBT religious bigots. He has signaled support for the First Amendment Defense Act, the vilest anti-LGBT”religious freedom” bill of our time, which would allow businesses and individuals to discriminate against gay people in the guise of ‘religious liberty’.

Also, Trump used the massacre of 49 people at Pulse nightclub in Orlando to promote his bigoted immigration policies and claim to be a better friend to women and the LGBT community than Hillary Clinton. Just days before the massacre Trump told a group of religious right leaders that he would defend “Christian values” including “marriage and family.” He joined Marco Rubio at an anti-LGBT event on the two month anniversary of the Pulse massacre.

So don’t be fooled by this fraudulent, pandering display.

London Clinic Says PrEP Bought Online is Legit

October 31 2016

__________________________________________________________________________________

A health clinic located in central London called 56 Dean Street decided to test drug levels in users that bought generic tenofovir/emtricitabine (PrEP) online to see if what they had contained adequate levels of both drugs in their blood. Turns out, they were legitimate.

The data was presented by Dr. Nneka Nwokolo, who works at the clinic, at the International Congress on Drug Therapy in HIV Infection (HIV Glasgow), reported by AidsMap. The study itself was spawned after discovering an increasing number of patients coming in for routine tests had been buying generic PrEP online — mainly Cipla’s Tenvir-EM, Emcure’s Tavin-EM, and Mylan’s Ricovir-EM.

“This is an important collaboration between clinic patients, community activists and clinicians — it is a verification that online pharmacies are genuine and results can be made available, and adds to the testimonies from PrEP users,” Dr. Nwokolo said. “As long as PrEP is unavailable from the National Health Service, it is crucial that it is accessible to those who need it.”

Dr. Nwokolo and her physicians compared tenofovir and emtricitabine levels in the blood from patients using PrEP from the generic brands above, and those who took Truvada, which was supplied at 56 Dean Street.

The results found a regular tenofovir level of 103 ng/ml and a emtricitabine level of 142 ng/ml. Twelve of the patients were tested twice with the same results. And within the first twelve hours, physicians found regular or higher levels seen in patients who took drugs supplied by the clinic. Even better, they found kidney function to be totally normal in all those tested.

Even though follow ups were usually about three months long on average, research showed zero HIV infections in all patients — less than the PROUD study, which after two years found 9 percent of infections among patients. A follow up is planned to take place around STI, and hepatitis B and C infections.

Why do Uncut Guys Get Blamed for Spreading HIV?

October 30 2016

__________________________________________________________________________________

Stigma can destroy a perfectly good penis, even when it looks, feels, and acts like a divine stalk of pleasure at your fingertips. Whether we like it or not, the foreskin comes with a lot of ideas attached to it — and not all of them are good.

The growing case for circumcision has been reported by Plus before. While several types of research (mainly in Africa) have shown it reduces the risk of HIV transmission through penile-vaginal sex, as the Centers for Disease Control and Prevention says, studies determining whether circumcision reduces HIV transmission, either to female or male partners, indicate mixed results.

During the AIDS epidemic, nearly 85% of adult men were circumcised, Psychology Today reported, and yet HIV still spread. According to PT, the three studies in Africa claiming that circumcision prevented HIV had many flaws, including that they were stopped before the results came in. Unfortunately these studies gained publicity around the world, even a widespread belief in Africa that if you’re circumcised you don’t need to wear a condom. In fact, Plus reported that Kenyans lead the world in voluntary male circumcisions to prevent HIV (40,000 in December 2010 alone).

Plus also reported about a study in The Journal of the American Medical Association, showing that gay guys who were circumcised were 14 Percent less likely to be HIV-positive than those who were not. But to use circumcision as a form of prevention doesn’t make sense. As CDC researcher Gregorio Millett pointed out, there isn’t “enough data out there right to now to show whether there’s a benefit.”

But let’s get back to the stigma. How did it begin and why is it here? For a while, uncut men were fighting against an idea that foreskin is unclean, prone to spreading infection, and even more disturbing, totally un-American. After all, circumcision is a time honored American tradition, but have we been changing our tune in the last few decades?

Figures from the National Center for Health Statistics at the CDC show that rates of circumcision in America dropped ten percent in the last thirty years. In comparison to the nearly 70% of males in the world who are uncircumcised, the United States is one of the leading countries in the world who choose to circumcise their male babies. So is it a lack of information that fuels stigma around foreskin, or is it merely cultural?

At the end of the day, misinformation is dangerous when it comes to health. Whether you’re cut or uncut, there is one thing that will always prevent STD transmission: practicing safe sex. No amount of extra skin on a penis can stop you from wrapping it with a condom. We already know that. So let's stop the stigma.

Rimming, Flesh-Eating Bacteria, Drug-Resistant Gonorrhea, and More

October 31 2016

________________________________________________________________________________

This article originally appeared on Beta Blog.

What infections can be spread by rimming?

Nicholas Moss, MD, MPH: There are a few different types of infections that can be spread by rimming. First, infections that can be transmitted from skin to skin contact (like syphilis) could be spread by rimming. Syphilis bacteria live in active skin sores, or lesions, and can infect other skin it comes in contact with. I’ve even seen one patient who had a syphilis sore on his forehead. People can get gonorrhea of the throat, so that’s another infection that could be spread by rimming. Chlamydia of the throat is not spread orally, to my knowledge. So there’s probably no risk of getting or transmitting chlamydia when rimming.

Also, it’s possible to get other kinds of infections when you come into contact with fecal material. It’s possible to get hepatitis A, and infections like diarrheal illness and food poisoning if you come into contact with someone who has recently been infected with something like salmonella.

What’s going on with drug-resistant gonorrhea?

I think we’re headed to a world where we have strains of gonorrhea that we just aren’t able to treat. Over the last 70 or so years that we’ve been using antibiotics to treat gonorrhea, the bacteria has gotten better and better at beating the drugs we’ve used to kill it. “Drug resistant” just means that the bacteria has evolved to the point where certain antibiotics no longer work against it. So far, we’ve had a few cases of people with gonorrhea that’s extremely difficult to treat, but it’s very rare. Now, we routinely use two drugs at the same time to treat gonorrhea to make sure that we get rid of the infection.

How do you know if you do get drug-resistant gonorrhea?

We ask people to wait about a week for their gonorrhea treatment to work. If, after a week, you still have symptoms then you should give your doctor another call.

What are chlamydia symptoms to look out for?

If you have a chlamydia infection in your urethra, you might have pain or a burning sensation when you pee. Or, you might get symptoms around your rectum, like pain or discharge. It can feel like constipation. There’s one type of chlamydia called LGV where men get swelling in their groin creases and the lymph nodes that are there. So if you have soreness there, and you’re feeling sick or have a fever, it’s time to see your doctor.

I heard that shaving your body hair makes you more likely to get skin infections. Is this true?

Bacteria that causes skin infections, like MRSA, can get into and live in tiny cuts in your skin (that you might not even be able to see). Certainly things like shaving your hair can give you more of those cuts on your skin, which is probably why you’ve heard this. But I’d say you should balance this with how you want to live your life. If you’re somebody who gets a lot of skin infections, then it might be worth not shaving your body. But otherwise, if you practice good hygiene then shaving your body hair is probably not something you need to worry about.

What kinds of problems do flesh-eating bacteria cause?

Flesh-eating bacteria is what people say colloquially to refer to bacteria that cause infections like staph (staphylococcus) or strep (streptococcal). These are very rare, but not so rare that I’ve never seen them. These kinds of infections need to be treated right away. Usually, a surgeon will need to cut out the infected skin and the person will be put on really strong antibiotics. Other simpler skin infections caused by staph or strep bacteria can cause pus-filled lumps or abscesses. These can be anywhere on the body, including in the butt and in places like your armpits. These have to be drained, and the person is also put on antibiotics. To prevent these kinds of infections, practice good hygiene like washing with soap and water.

What is cellulitis?

Cellulitis is a type of infection that can be caused by different types of bacteria. It’s an infection of the skin, and it usually spreads. It causes the skin to be red and sore to the touch. We often see it in the legs—it can be caused by athlete’s foot that’s not really well treated. Bacteria will get into a crack in the skin caused by athlete’s foot, and the infection will spread up the leg. Usually, it’s caused by the strep bacteria, and can be treated with antibiotics.

Is the over-use of antibiotics in agriculture responsible for drug-resistant “super-bugs” like MRSA?

I don’t know if there are specific instances of drug resistance developing in direct response to the antibiotics used in agriculture. But most infectious disease doctors are against the use of antibiotics in agriculture. Just putting antibiotics out there in the environment, unnecessarily, increases the odd that we’ll get these resistant types of bacteria.

Are people living with HIV more likely to develop these kinds of skin infections?

It’s possible that people who have a compromised immune system may be more likely to develop certain infections. But really, it’s hard to know what the effects might be for long-term survivors or others living with HIV. So many of these infections we’ve been discussing are common. So if you have an undetectable viral load, if your CD4 cell count is over 500, if your immune system is in good shape—you maybe will get one extra cold or flu every year. But otherwise you’re going to run the risk of getting these skin infections like everyone else. If you want to protect yourself—opt for good hygiene. Choose the level of risk that you want in your sex life. And if you don’t feel well, go see your doctor. Get things that crop up looked at by a medical professional.

All Hail Prince Harry's Battle Against HIV Stigma

October 30, 2016

________________________________________________________________________________

Over two million people watched as the U.K.'s Prince Harry (the late Princess Diana's youngest son) appeared on Facebook Live earlier this year to show how easy, quick, and painless it is to get tested for HIV — and to support the Terrence Higgins Trust, the largest non-profit organization supporting people living with HIV in Europe. Since then orders of the rapid HIV test have increased 5,000 percent.

“It’s so important that we continue looking for new ways to make HIV testing more accessible to those most at risk,” Dr. Michael Brady, the medical director for the Terrence Higgins Trust, told reporters. “[That’s] why it’s fantastic to see the very tangible and immediate impact of Prince Harry’s support for HIV testing.”

A similar impact was seen in the U.S. when Charlie Sheen came out as HIV-positive, demonstrating the power of celebrities to raise awareness among the general public. The Charlie Sheen effect didn’t just bring about an increase in testing. It also caused a huge increase in HIV reporting.  A study conducted by Seth Noar of the University of North Carolina School of Media, and published in JAMA Internal Medicine, an American Medical Association journal, discovered that Sheen’s disclosure caused a 265 percent increase in news reports mentioning HIV (97 percent of which also mentioned Sheen).

Let’s hope more celebrities will follow in the royal footsteps, publicly getting tested and calling on fans to know their status.
